  1. I feel that Dr. Miller is right in his assertion that we as healthcare providers have a great influence on the success of any health behavior change. This is because the approach that we choose will greatly determine whether the client or patient will be motivated to change or not. For instance, a confrontational approach to health promotion will only push the client away and make them revolt against any advice that you give (MerloLab, 2009a). According to the transtheoretical model (TTM) of change, a client or patient will go through a continuum of phases that involve concious decision making to change (Duran, 2003). In order to encourage the patient to start on the first of these phases, the provider has to be less confrontational and more motivational (Columbia University, 2009). The so-called “in your face” counseling referred to by Dr. Miller could be labelled as a confrontational and combative approach to counseling. It is the attitude adopted by the provider that tells the patient/ client “adopt change or perish” and is premised on the idea of “pushing for change.” However, this approach to health education and promotion is counterproductive in every sense (Nichols & Davis, 2017). A good and effective health promoter and educator is motivational, empowering, and encouraging. They are never judgmental (MerloLab, 2009b).
